This plan is the last step in a three part planning process to address the needs of our community. The agency’s strategic plan builds off its predecessors, the Community Health Assessment (CHA), and the Community Health Improvement Plan (CHIP). The strategic plan will guide the decisions of the health department’s staff and stakeholders over the next few years by devoting time to achieve the goals and objectives we have set forth in this document.
