“In order to better understand the needs our population we serve, our health department conducted a food availability and physical activity survey with our senior population. As part of our health department’s Community Health Assessment, we wanted to collect additional data surrounding fruits, vegetables, and physical activities as there was a high need in these areas. Chronic disease was identified as a priority for our community members. This survey is used to aid our understanding and identify risk factors associated with chronic disease. Below is the summary of our primary quantitative data”.
